Look Globally - Act Locally: Integrating Care in the Community for Vulnerable Populations

Event Details

This symposium, presented by the Canadian Research Network for Care in the Community (CRNCC) and the Ontario Community Support Association (OCSA), featured top speakers from Canada and from European countries such as Austria, the Netherlands, and the United Kingdom. These countries have implemented or are currently implementing integration initiatives.  A key goal was to learn more about what works, what does not, and what needs to be done at the local level to implement successful integrated client care initiatives. See below for speaker's bios and presentations!


This symposium also featured a “town hall” panel discussion which aimed to surface integration challenges and opportunities currently experienced by participants including front-line providers, consumers and policy-makers.  While integration challenges are global, solutions are local.

Background


Canada's aging population, rising institutional care costs, persistent shortages of health human resources, concerns about the sustainability of health care and changing values to age at home have pushed Canada to consider comprehensive continuing care approaches to health and social care. What is the balance of evidence regarding continuing care initiatives in Canada? What are the policy and practice implications for continuing care when older people want to stay at home?

The Ontario Community Support Association (OCSA)

The Ontario Community Support Association (OCSA) supports, promotes and represents the common goals of its members. They provide not-for-profit health and social services that help people live at home in their own communities, across Ontario.
OCSA represents more than 360 community agencies, 25,000 staff and 100,000 volunteers across Ontario. Last year these volunteers donated over 2.5 million hours of service. OCSA works with their members to ensure that issues which affect community services are heard and addressed by government policy makers, other health organizations, the media and the general public.
